Dr Scott Boltz

 

Dr. Boltz originates from Greenwood, Indiana and developed an interest in dentistry as a young boy. He began working as a dental assistant on weekends in 1974 while in High School.

Dr. Boltz graduated from Center Grove High School in 1976 and did his undergraduate education at Purdue University from 1976 - 1979 earning a Bachelor of Science in 1980 in Biology. He attended Indiana University School of Dentistry receiving his Doctorate of Dental Surgery in 1983. During the 4 yrs of dental school, thought was given to specializing in oral surgery. By the end of dental school, Dr. Boltz realized there were too many facets of dentistry he found enjoyable and rewarding to eliminate by specializing in just one field. Following dental school he completed a one year Hospital General Practice Residency at Regenstrief Health Center, Wishard Memorial Hospital in Indianapolis. This allowed him to have additional training in oral surgery, periodontal surgery, sedation and anesthesia. Dr. Boltz taught part time as an associate professor at the Regenstrief General Practice Residency from 1984 - 1985.

Dr. Boltz began practice as an associate dentist in the Kokomo area in 1984 and opened Family Dental Center with friend and partner Dr. Ray Christine in June of 1991. He has worked as a volunteer dentist at the Kokomo Clinic of Hope and Dentists Delivering Smiles program and looks forward to more mission dentistry opportunities in the future.

Dr. Boltz is blessed with a wonderful wife, Jayne, and three children, Becky, Laura and Nathan. He enjoys family time, serving his church in music ministries, and bicycling.

Licensed Dental Hygienist


Christy is originally from Kokomo Indiana. She has been associated with Dr. Boltz since 1991. She earned her Associates of Science degree in Dental Hygiene at Indiana University - South Bend in 1988. Prior to hygiene, she served 6 years in the dental profession as an assistant. In her spare time she enjoys taking care of her family. She is the mother of 3 boys and their family attends Alto Road Church of Christ. She promises to be gentle as she helps you to fight dental disease.

Licensed Dental Hygienist


Tammy is originally from Walton Indiana. She earned her Associates of Science degree in Dental Hygiene at Indiana University School of Dentistry 1989. She started her association with Dr. Boltz in 1994. After a few years sabbatical from full time hygiene to be home with her children she returned to work two days a week with our team. In her spare time Tammy enjoys her family, exercising and reading. She promises to be gentle as she helps you to fight dental disease.

Expanded Functions Dental Assistant


Linda comes to us from Peru. She has been with Dr. Boltz’s team since 2002 after a sabbatical to be home with her small kids. Linda earned her raidology and expanded duties certifications from IU School of Dentistry. She has been assisting in the dental field since 1984. She participates in continuing education opportunities to further her skills. During her free time she enjoys swimming and camping with her children.

Expanded Functions Dental Assistant


Kimberly is from Greentown Indiana. She joined Dr. Boltz‘s team in September of 2003. She earned her Expanded Functions Dental Assistant Cerfication in 1994 at the IU School of Dentistry, and brought with her, 13 years experience in the dental profession prior to joining us. In her spare time she enjoys being with her husband James and her family. She also enjoys working at their horse farm in Greentown, trial riding, camping as well as attending the kid’s sporting events and attending her home church, Jerome Christian in Greentown.

Back Office Coordinator


Sandy is from the Galveston area and has been with Dr. Boltz since 1999. She oversees the office infection control procedures and the sterilization of all dental instruments, maintaining OSHA & CDC (Centers for Disease Control) standards. She helps to coordinate and assist in patient care for the doctor and hygienists. She feels blessed with a great family. Her three children and one grandson keep her very busy. She also enjoys running and bicycling.
